The U.S. Department of Labor (DOL) has released updated versions of its model Family and Medical Leave Act (FMLA) certification and notice forms. The new expiration date is August 31, 2021.

While the new forms are virtually identical to previous versions, they serve as a useful reminder for HR professionals to maintain up-to-date forms for all employee leave and benefit use. For organizations utilizing customized FMLA forms, rather than those issued by the DOL, this is a great opportunity to ensure all company-specific documentation matches the DOL-issued forms. This is also a great time for employers to review their FMLA policies, as well as any “cheat sheets” used by HR professionals for employee leave procedures, to ensure compliance with FMLA requirements.
