All employers must report serious workplace incidents that result in amputation, loss of an eye, inpatient hospitalization, or death to the Occupational Safety and Health Administration (OSHA). Other annual reporting requirements can apply in less serious incidents based on industry type and employee count.
OSHA 300 logs are required for most employers. Employers must record, with some exceptions, work-related injuries and illnesses on the OSHA Form 300 log within seven days. There is an exemption for employers with fewer than 10 employees and employers in very low-risk industries. Information collected in the OSHA 300 logs is then used to complete Form 300A. The summary must be signed by a company executive and posted in the workplace from February 1 to April 30 of the year following the reporting year. The reporting deadline to electronically submit 2024 data to OSHA is March 2, 2025. OSHA’s ITA coverage application is available to help employers determine their electronic reporting requirements.
Employers subject to Form 300A
- 20-249 employees in high-risk industries such as construction, trucking and utilities,
- 250-plus employees, unless listed as an exempt industry,
- 100-plus employees in industries with elevated injury and illness rates. (Also must submit Forms 300 and 301).
The OSHA 300 Log and the OSHA 300A Summary must be retained for five years after initially completed. OSHA’s ITA coverage application is available to help employers determine their electronic reporting requirements.
